Declaration form should be signed by the ADULT and no parent needs to be present. Parental presence only refers to an application for a MINOR.

Birth Certificate must clearly state applicant’s Belizean parent/s to show who the nationality is from.

Copy of nationality certificate

declaring the reason for the replacement certificate signed by the applicant in the presence of a Justice of the Peace or Commissioner of the Supreme Court, Attorney at Law, Notary Public, Solicitor or Attorney who musts also sign and date the declaration. For replacement of lost certificate: Declare the circumstances surrounding the lost certificate and request a replacement. Police report, if available. For other replacement of outdated or mutilated certificate: Declare the reasons for the request of a replacement.

2 inches by 2 inches. Taken within the last 6 months; left blank at the back.

If applicant does not have a passport from another country, the original birth certificate MUST be apostilled in the country of birth or at the Foreign Affairs/Exterior Ministry or Embassy of the country of birth.
